- introduction
- There are 15% and 40% relapse rate in patients after archiectomy and radiotherapy with non-bulky stage II (IIA+IIB) and with stage IIC respectively. Different combined treatment protocols are tested in many centers.
- material and methods
- The retrospective analysis of 85 patients treated between 1985 and 1997 was performed. The age was between 22 and 59 months, median diameter of tumor was 7cm (1-20). After orchiectomy chemotherapy was administered: median number of cycles was 3 (1-6), PVB – 32 pts, BEP – 34 pts, CEB – 6 pts, Carboplatin – 12 pts, 1pts VAB – 6. After chemotherapy radiation treatment was given with median dose 30,5 Gy (11-50 Gy), radiation fields included paraaortic and ipsilateral iliac lymph nodes, 1 patient had mediastinal field and 1 pt supraclavicular field.
- results
- In the median time of observation 90,7 months (2-181) 90,6% of patients are alive and disease free, 8 pts (9,4%) died, 6 of them (7,1%) in the course of disease progression (PD). PD was observed in 9 pts (10,6%), time to progression was 6-58 months. Treatment toxicity was acceptable, most frequently anemia and nausea were seen, radiation tolerance was good except one case - toxic deaths in course of RTH due to significant radiation overdose.
- conclusions
- Combined treatment (chemotherapy and radiotherapy) in patients with stage II seminoma is highly effective and feasible.
