summary

The authors evaluated 400 adenomectomies performed in the years 3978?1986. In 29 patients (7,3%) neoplastic texture in the adenoma was found. Besides, in 169 patients (42,3%) the exanimation revealed chronic and in 53 (13.0%) purulent prosta-tis. Before admitting to hospital, 249 patients had been catherized and 224 of them (90.0%) had the catheter left. The complication developed significantly more often (p<0.001) in the patients operated who had been catherized (temporarily or per-manently).
The mass of adenoma was on the average 60.6?38.6 g. There was a clean tendency towards an increase in the mass of adenoma in older patients. No neo­plastic texture was found in any of the adenomas of more than 120 g.
Postoperative complications appeared in 63 patients (15.7%), among them 58 patients also had infection in adenoma diagnozed (92.1 of complications). Three patients died after the operation (0.75 per cent).
Patients with smali adenoma are to be considered to be at risk of developing cancer. Catheterizing or leaving a catheter for long leads to inflammatory changes in many patients. The older a patient, the larger is the mass of adenoma, which indicates to the necessity of possibly early surgery in patients with prostatic ade­noma and giving up conservative therapy that is not very effective.
