Introduction. In spite of development of radiotherapy and other organ sparing procedures, radical cystectomy has been still a „golden standard\" in the treatment of invasive bladder carcinoma. An urinary bladder replacement has been still a challenge for urologist. There a lot of techniques of bladder replacement. The aim of this study is a clinical estimation of the Alcini neobladder after three years of the use of this technique.
Material and method. Between 1998 - 2000, seven patients underwent radical cystectomy with Alcini neobladder due to infiltrative bladder cancer and one patient underwent this operation due to postradiation bladder. After surgical treatment, in all patients intravenous urography, cystography, ultrasonography and urodynamic examination were performed.
Results. In six patients imagine examinations showed normal kidney function without uretcro-bladder nor bladdcr-urcthral strictures. Three patients demonstrated vesico-ureleral reflux without clinical symptoms and signs. In all patients lower urinary tract infection was observed. Mean maximal contraction pressure was 54.7 cm IbO, mean maximal uroflow 18.2 ml/s, and mean bladder capacity 360 ml. In three patients abdominal press was used during micturiction. One patient after radiotherapy underwent bilateral nephrostomy due to bilateral hydronephrosis 12 months after cystectomy.
Conclusions. In some anatomical conditions, the Alcini neobladder is a good method od supravesical diversion because of a good function, simple operative technique and a short operation time.
