Severe penile injuries: etiology, management and outcomes

Introduction and the aim of the study.
Penile trauma is difficult physical and psychological problem and its reports are sporadic. Their type and extension vary in severity from small to more serious injuries and total emasculation. We review our experience in patients with severe penile injuries and mode of their treatment.
Materials and methods.
A retrospective analysis of 34 patients with severe penile injuries referred to us from March 1999 to August 2004 was undertaken. Etiology was different as well as procedures for their treatment. Approach was individualized according to the case.
Results.
The follow-up ranged from 6 months to 6 years (mean 2,8 years). The causes of penile injury in these series were traffic accidents (14), iatrogenic trauma (13), self-amputation (2), electrocution (1), burns (2) and gun shoot trauma (2). Good aesthetic and functional results, including satisfactory sexual intercourse, were found in 30 cases (88%). Complications occurred in four cases: one infection after penile prosthesis implantation and three urethral complications, one stenosis and two fistulas.
Conclusions.
Severe penile injuries should be treated on a case by case basis using different techniques. However, it is effective and safe only in hands of an experienced surgeon who is highly familiar with penile reconstructive surgery and who is permanently in training.
