introduction and aim of the study
Chronic prostatitis/rostatodynia are one of the most common conditions in urology. Their chronic and irritating symptoms and relapsing nature pose a serious diagnostic and therapeutic challenge for patients and doctors alike. The author presents the latest knowledge on etiology of chronic prostatitis/prostatodynia and treatment using alpha adrenoceptor antagonist.
materials and methods
This paper is a review of the latest urological references related to diagnosis and treatment of chronic prostatitis/prostatodynia. He pays particularly close attention to the usage of alpha blockers as the treatment of these diseases.
conclusions
Escalation of lower urinary tract symptoms of these diseases can reduce the quality of life for men more than those in benign prostatic hyperplasia (BPH). The presence of prostatitis and the symptoms of prostatodynia can be closely linked with high intra-urethral pressure as well as with reverse reflux into the prostatic tissue. Open and randomised prospective studies presented the possibility of treating patients with chronic bacterial and non bacterial prostatitis and prostatodynia with alpha adrenoceptor antagonist. Simultaneous usage of antibacterial and anti inflammation drugs can increase efficacy of the treatment. This treatment is particularly beneficial to patients with confirmed disorder of urinary flow with inflammation of BPH. The usage of alpha blockers in order to prevent recurrence of prostatitis and prostatodynia in relation to the etiology of reverse reflux into the prostatic tissue seems to be fully justified. Bearing in mind their side effects the dosage of alpha adrenoceptor antagonist in patients with prostatitis is identical to those with BPH. For this reason prescribing uroslelective tamsulosin is recommended, especially in young men. It is generally believed, that the duration of administering alpha adrenoceptor antagonist should not be less than four weeks. As a preventive measure it should be administered for six to eight months.
